Population health, primary care networks and personalisation: Using data to transform care and improve health outcomes
Ryan Irwin is a population health strategy executive at Cerner with a particular interest in primary care quality improvement and population health management. In this article, Ryan shares key ideas on how health care organizations need a sharpened focus on population health with the global onset of COVID-19. Population health management will only deliver a step-change in health and care delivery if care is personalized to the individual and their unique context. Central to the delivery of personalized care within new models of care is the use of evidence-based, person-reported outcome measures (PROMs) like PAM® that can be used within wider data sets to support risk identification and assess health outcomes from the perspective of the individual.
