Wednesday 5 September

10:15 – Dr. Ollie Hart, Sloan Medical Centre
Ollie Hart is a GP partner in Sheffield at the Sloan Medical Centre. He is the clinical lead for Sheffield’s person-centred care work stream, and a founder member of the Sheffield Move More board. He believes in creating systems that make it easier for people to be active and activated.

1:30 – Rachel Gair, UK Renal Registry
Rachel works closely with the clinicians and patients as the Person-Centred Care Facilitator for the Transforming Participation in Chronic Kidney Disease programme. She is also the Expert Nurse Specialist on the Programme Board and a Registered General Nurse and trained at Hammersmith Hospital in London.

Thursday 6 September

At the Home Group, Naomi was a Health and Social Care Partner directing Social and Public Health programmes, including a demonstration project utilising Insignia Health’s Coaching for Activation® (CFA®) with individuals receiving coaching services from the Home Group’s staff.
