Without a systematic way to assess patient capabilities for self-management, coaching often takes a one-size fits all approach that pursues evidence based self-management objectives for all participants. Coaching for Activation® takes a different approach based upon a PAM score. Tailoring support to activation levels recognizes that individuals possess differing levels of knowledge, skill and confidence in managing their health. Coaching for Activation meets the individual where he or she is, allowing for education, health goals and action steps to better engage and move individuals along a path toward higher activation.

Skill development becomes increasingly complex as individuals pursue best practice self care at the higher levels of activation.
