Insignia's Survey System is a comprehensive application encompassing survey administration, reporting, analytics and data warehousing. Information gathered in the system is easily transferred to other data systems.
The Survey System supports flexible and tailored administration of the Patient Activaiton Measure™ (PAM™) self-assessment tool. Administer the survey via paper, over the phone, or let participants self-administer via an online interface. In addition to the PAM survey, the Survey System provides additional batteries to aid in collecting valuable participant information, including a health status and functioning inventory, depression screening, and demographic questionnaire.
Reporting & Data Exporting
The Survey System contains robust reporting features for individual and group analysis. Individual reporting reflects responses to each survey question, the activation level and score for each administration of the survey. Track the participant over time to monitor progress toward goals. When used, the health status and functioning responses integrate into the PAM assessment reporting.
