Insignia Health - Health Plans

Insignia's PAM-based programs for coaches (Coaching for Activation®) and for members (Flourish™) bring activation scores to life, and are easily customized to integrate with existing health plan resources.

  • Strengthen behavior change through disease management and prevention efforts
  • Customize online and print communications for greater impact
  • Tailor incentives to maximize return

Improving Health, Controlling Costs

It’s no mystery what employers are seeking from their health plans: cost control and improved employee health. We help health plans make progress on these efforts with disease and case management, wellness programs and a host of online and print member educational resources. Often, these types of programs are tailored to behavioral deficits — eating too few vegetables and fruit, not taking medications as prescribed, lacking exercise, as revealed by health assessments and claim data. This approach lacks an empirically derived understanding of the underlying self-management competencies that are driving poor behaviors and escalating healthcare costs. The Patient Activation Measure® assessment takes member support and education a major step forward by revealing the competency-based drivers of these deficits, and allowing resources to be tailored to each member’s level of activation.
